Driving Directions:

• From Hillcrest Road you can turn West onto Hillcrest Plaza Drive. 

• From the eastbound frontage road to 635 you can turn South onto Hillcrest Plaza Drive.

• The building is a tan brick and beige building that is two stories backing up to a creek. You will see a sign on it that reads Concorde on the Creek.

Once you arrive:

• Inside, you will go upstairs (Suite 308 is the second/top floor) If you take the stairs in the main lobby, turn to your left, go as far as you can and then turn left again.  Suite 308 will be on the right side.  There is an elevator across from the stairs.

• In her waiting room, you will see a computer screen. Select her name form the list of providers, and then follow the instructions so she knows when you arrive.
